There was some discussion in a Facebook group I’m in about this commercially available card. After going back and forth for a bit, explaining how it’s made, I sat down and actually MADE IT. Roughly 
Cutting files, instructions, more pictures…
I probably spent more time fiddling with the slots, to optimize their locking-together power while not cutting through the top letters, than any other part of the design process.
It’s a pretty standard sliceform, set diagonally into the card.
You can have it angle this way if you prefer.
